Event Application

To submit your university sponsored activity or event to the Passport Office for possible inclusion in the 2003-04 Passport to Success program, please fill out this brief form and send it to: Passport Office, 333 Wells Hall, Murray State University. Applications for new events must be received at least one month prior to the event. The selection committee will meet the first of each month to review applications for the next month.

Program Tracks

Events and activities submitted for approval should fall into one of the following tracks:

  1. Personal Growth: Programs and structured activities designed specifically to enhance personal growth and student success.
  2. Citizenship and Outreach: Programs that provide insight into citizenship or provide service and communication with the greater Murray community.
  3. New Horizons: Topical programs that expand knowledge in a specific academic discipline not included under the other headings.
  4. Fine and Performing Arts: Programs and performances in the fine and performing arts.
  5. Life-Long Learning: Students may select from any event offered in the Passport Tracks, or additional activities approved by the Passport Office.

Programs and activities selected for the 2003-04 Passport to Success season must agree to meet the following stipulations:

  1. Events should be at  minimal cost for students.
  2. A representative of the event sponsor will pick up student attendance cards from the Passport Office, 333 Wells Hall, and make certain that the cards are distributed to students as they enter the event.  Furthermore, a representative of the sponsor will collect the attendance cards as students leave the event.  It is imperative that students receive and deposit only one card, thus, cards may not be left unattended for distribution or return.
  3. Attendants or ushers will not accept attendance cards from students who leave the activity or event early.
  4. Following a Passport event, completed and unused cards must be delivered to the Passport Office before 4:00 p.m. on the next working day.
  5. If the date, location, time and/or content of the event changes, the Passport Office will be notified at once.
